Radiofrequency microneedling is one of the few treatments where clinics openly quote two prices for the same machine — one where a doctor performs it, one where a trained therapist does under the doctor's direction. That is not a trick. It reflects a real distinction in what may be delegated, and it is worth understanding rather than assuming the higher number is padding.

Why there are two tiers

Some indications and some settings call for a doctor to perform the treatment. Others may be delegated. Both are assessed and planned by the doctor. Which applies depends on what is being treated and how aggressively — and a clinic should tell you which one your plan uses before you book, not after.

Needle depth and settings

The depth the needles reach is adjustable, and deeper treatment does more — for acne scarring in particular. It also hurts more and involves more downtime. A gentle, shallow session and a deep scar-focused session are different treatments on the same device, and the price follows the intensity.

Disposable tips

Each session uses a sterile single-use tip. This is a genuine per-session consumable cost and it is a substantial part of the price. It is also non-negotiable — a clinic economising here is not one to choose on price.

Area and add-ons

Neck and décolletage are usually quoted separately from the face. If the neck is part of what bothers you, get it into the quote from the start.

Course length is the real cost

Meaningful change — particularly for acne scarring — comes from a course, commonly around four sessions spaced a few weeks apart, then reassessment. A single-session price is a poor guide to what the outcome will cost you. Ask what a realistic course looks like for your skin, and what the clinic expects to achieve with it.

Downtime, honestly

This is one of the more uncomfortable treatments even with numbing, and it is worth knowing that before the day rather than during it. Expect redness and a warm, sunburn-like feeling for a day or two, with fine pinpoint marks visible for a short period.

Topical numbing also adds waiting time to the appointment beyond the treatment itself — a "sixty-minute treatment" is usually closer to ninety minutes in the clinic.

A note if you have melasma

Melasma can be unpredictable with any energy-based treatment and can worsen with aggressive settings. If you have it, that needs to be part of the conversation before a course is planned — not discovered afterwards.

• What needle depth, and what is being targeted?
• How many sessions, at what interval, and when do we reassess?

Where it will not deliver, at any price

Deep, narrow ice-pick scars and heavily tethered scars generally need procedural techniques that energy devices cannot replace. A course of RF microneedling will improve the surrounding texture and disappoint you about the pits. That is worth hearing before you buy the course, not after.
